About Your Patient Portal

 

When you’re scheduled for an appointment as a new patient, you’ll receive an email with a link to the portal. Follow those instructions to set up your account. The first time you access your portal, you must use the link sent in your email. It expires after a few days so call if you need another link! When you log in, you’ll sign the new patient paperwork and check in for your appointment at home on your own schedule.

Some of the things you can do in the portal :

  • Check-in for your appointment & complete your medical history & medications at home on your own schedule!

  • Sign new patient paperwork.

  • Message the physician & medical assistants for non-urgent issues.

If you haven’t logged in before, use the link in your email. Call us if your link has expired. If you’ve lost your username and password, don’t worry! You can login using a verification code that will be sent to your cell phone if we have that number in your chart.

 

Forgot Your Username or Password?

There are two ways to login to your portal.

The first way is with your username and password you would have set up when you scheduled your first appointment. If you’ve forgotten your username and password, you can login by getting a verification code on your cell phone. We have to have your cell phone number in our system in order for this to work. See the instructions for both below.


1) Log in with Username and Password

1.     Enter your Username.

2.     Enter your Password.

3.     Click Log In.

  • ·       You are allowed three login attempts before your account is deactivated. 

  • ·       If your account is deactivated, you can still log in using a verification code.

  • ·       To avoid account deactivation, click Forgot username or password? to request your username or reset your password.


2) Log in with Verification Code

Make sure we have your cell number registered in your chart. Proxy users cannot use this method.

 1.     Click Log in with verification code.

2.     Enter your First Name.

3.     Enter your Last Name.

4.     Select your Date of Birth.

5.     Enter your Mobile Number.

6.     Click Send verification code.

The verification message has been sent to your phone.

7.     Enter the verification code.

The code expires in three minutes. To have another verification code sent to your phone, click Resend verification code.

8.     Click Log In.

You are successfully logged in to the Patient Portal
